The 'This child is eligible to participate in the program' field does not check off yes or no. Enrollment has to print this form, sign and rescan into Child Plus. But some fields are not being automatically checked off.
Also, can you add enrollment specialist for our title.

I have added Enrollment Specialist in the title. Please let me know if it is still coming up blank under the title space on the eligibility verification form.
I am still working on finding out why the check-mark isn't appearing when an applicant is eligible. For now, please manually check off yes.
Also, remember that in order to pull all correct information on the eligibility verification form please - check off 'Print Eligibility Verification Form', Child Participant, and Which enrollment record (three check marks total) - only print the page number of the Eligibility Verification Form
If information is missing. Try the following based off of section on form:
1. Verify that Full legal name is correct, if not change under participant's application screen
2. Child's date of birth is missing or incorrect, find it under participant's application screen
3. ****STILL FIGURING THIS OUT, Manually check yes after printing form for now
4. Only one initial box will be checked (options are Income, Over-income, Public Assistance, SSI, Homeless, or Foster). If Income or Over-income is checked, then second box under income/over-income will also be checked with explanation.
*If 4. is missing a check-mark...verify that income information from family information screen in application field transferred over to enrollment. (income eligibility amount, type of income, SSI, TANF)
****IF APPLICATION WAS NOT ENTERED ALL AT ONCE FROM START TO FINISH, INFORMATION WILL NOT TRANSFER OVER TO ENROLLMENT SCREEN.
5. This will check off the option chosen for income verification under family information

Yes, please continue to manually check off the questions that asks if the child is eligible.
